How to make medical visa to Germany?

Technique of legalization of entry documents – visas in Germany is similar to the processing of visas for entry into other Schengen countries. And it all depends on how carefully documents are assembled by the applicant.

Consular Section of the Embassy of German checks the documents of the applicant, especially the readiness to return, and, of course must be clear indication the reason of any visit. Compulsory condition of the consular department of the Embassy of Germany – supply only the absolute complete package of documents, or in the examination you will be denied. Consular services of Germany must make sure in the aim of visit and the veracity of documents with the personal filing of the applicant.

1.Documents confirming the applicant’s identity:

  • Passport, which must still be valid for 3 month after the end of travel
  • Copy of the identification page of the passport
  • Three recently made passport photos, two of which should stick to the form, and one- submit separate
  • Original and a copy of your national civil passport (only pages that contain the records)

2. Form, also (2 copies, completed and personally signed forms. Fill the form in Latin letters according to passport data and other documents).

3. To explain the purpose of the visit you need following documents:
Confirmation from the clinic/ doctor who will treat in Germany, about the patient’s stay in the clinic and information about the planned duration and cost of treatment, and confirmation from the clinic about the implementation of prepayment treatment.

Please note that another formulation is not accepted!

4. Important! Provision of documents that confirm the applicant’s pay ability – acceptable credit cards, extracts from card account about cash flow or bank statements for the past three month (original and copies).

5. Embassy checks purpose of the trip and financial support for travel of accompanying person:
Confirmation of residence and credit cards (foe example, American Express, Visa, MasterCard and others – but do not accept debit payment cards such as Visa Electron)
And extracts from the card accounts about cash flow for the last three month, or bank statements for the last three month, or travelers checks (enough checks from calculation – 45 euro per night stay per person).

6. Embassy checks the medical insurance during the trip: health insurance coverage to the amount of not less than 30.000 Euro, valid for a stay on the territory of all Shengen countries throughout the schedule (in the insurance of long-term visa enough to file a medical insurance policy valid for first visit).

7. Embassy checks your rooting in the host country:

  • for employees - a certificate of job (including name, full address and phone number of the employer, indicating the telephone code areas, data of wage) or
  • to private entrepreneurs - documents of registration of private entrepreneurs and business enterprises (the certificate from the tax authorities, declarations, bank statements, accounting records, etc.) or
  • for students and pupils - a certificate from the institution or
  • certificate of property holdings

8. For children, under the age of 18 years, which is accompanied by only one of the parents or a third person:

  • a statement from father / mother of his/ her agreement to leave a child / children with notarized signature (original and copy) or
  • a judicial decision about the transfer of child’s right to one parent
  • or death certificate of one parent
  • or a certificate that a mother raising a child alone

Relevant documents are required to have while crossing the border.
