Visa to visit Switzerland for medical purpose.

For entry into Switzerland to citizens of Russia, Ukraine, Belarus, Turkmenistan, Uzbekistan, Kazakhstan, Kirgizstan, Moldova, Armenia, and Georgia required visa.

For those, who travel for medical advice or treatment required the following documents:

  1. A completed application form (in German, French, Italian or English) with the personal signature of the applicant (see “Visa Forms/ Photo requirements” on the site)
  2. A passport valid for at least three month from the date of departure from the Shengen area and having at least one empty page.
  3. Copy of passport (page with photo, name and signature)
  4. If available: a copy of the last two Shengen visas.
  5. Two identical, recently made by the Shengen sample photos: one glued to the form, the other attached.
  6. Medical and accident insurance, covering the entire stay and valid for the Shengen area (only copy). Insured amount must be at least EUR 30000.00.
  7. Confirmation of employment (the information from the employer) and income (wage) for students: student ID (original and copy).
  8. Well read copy of the invitation from the clinic or hospital with a doctor’s signature, confirming the fact of payment of the future course of treatment.
  9. Confirmation of the availability of transport in both directions (only copy).

For children under 18 years.
In addition to the list:

  1. Birth certificate (original and it’s notarized copy, translated into German, French, Italian or English)
  2. If the child is travelling alone should be given permission from parents to leave and it’s notarized copy, translated into German, French, Italian or English

In the case when the child is inscribed in the passport of Father/ Mother:

  • À separate, completed application form, signed by a parent, which is inscribed in the passport of a child
  • Two identical passport photos, made by the Shengen sample (one glued to the form, the other attached)- see "Types of visa/ photo requirements" of the site
  • Copy of passport (page with photo, name and signature), parent, accompanying the child
  • Copy of passport of parent where the child is inscribed
  • Birth certificate (original and it’s notarized copy, translated into German, French, Italian or English)
  • Medical and accident insurance, covering the entire stay and suitable for all Shengen area (only copy). Insured amount must be at least EUR 30000.00.
  • Documents regarded for at least three working days after the filing and payment of visa fee.

Embassy of Switzerland reserves the rights to:

  • require additional documents
  • send documents to the Swiss authorities
  • invite the applicant to the interview
